direct billing
At Main Street Massage Studio, I am pleased to inform you that I am able to direct bill to most of the Extended Health Benefit Providers. Please take the time to review this page for important details regarding the direct billing service.
I am able to Direct Bill to the following Extended Health Care Providers:
What I require from you at the time of booking your initial visit:
1. Insurance company name (Canada Life, Manulife etc.)2. Policy number & member ID3. Date of birth4. Primary member name & birthdate (Whose insurance plan is it?)
5. Photo of your benefits card for yout file
6. Please fill out the Direct Billing Authorization Consent Form and return it
Please Note:
-If you are insured with Manulife, you will need to pre-register on their website first, before electronic direct billing can be carried out.
-In the event that your insurance repayment cannot be assigned to the studio, you will be required to pay for the treatment in full at the time of check out. I will be able to submit your claim on your behalf for reimbursment to you.
-I am only able to submit to your primary benefits provider. Any remaining cost of your treatment that is uncovered by your primary provider will be required to be paid to the studio. I will provide you a receipt to submit the remainder to your subsequent or secondary provider if this situation applies to you.
-treatments under the Specialty Massages menu are ineligible to be submitted for reimbursement by Extended Health Benefit Providers
-It is your responsibility to know your available amount of coverage and keep track of available balances. Due to confidentiality, insurance companies do not share with us your balances.
-Some extended health benefit companies require a Physician’s Referral for Massage Therapy. If you are unsure if your plan requires this step, you will need to contact them to find out. If this is the case, you are required to submit the referral to your benefit company prior to your massage treatment date allowing for adequate processing time.
-Main Street Massage Studio is not able to issue Insurance Receipts when redeeming a gift card for treatment.
-It is against policy and considered fraud to issue a false receipt or change documentation or name on a receipt to be submitted for reimbursment.
